SILICONE GRANULOMA

silicone granuloma complicationThese days, many people are looking to improve their bodies through surgery or dieting or in other ways. There is a very alarming trend of patients getting injected with illegal products, including silicone. Silicone comes in many forms. There is the commercial silicone that you can buy at Home Depot, and there is medical-grade silicone that has been used to augment many areas of the body. I am sure you have heard about death secondary to black-market silicone injection that is routinely performed at hotels, as well as in other areas. If you survive the silicone injection, there are other complications that can occur, and these are very predictable. One of the most common complications is called a silicone granuloma.

Any time a foreign body is inserted into the human body, whether by injection or any implant, patients can develop what is called a granuloma. A granuloma is essentially a reaction of the body against this foreign object, and it creates a scar formation with inflammation around it. Now, if the granuloma is small, this is not a problem, but when you use silicone to augment the hips or buttocks, a significant amount is injected, and the patient can develop multiple granulomas. These can cause pain, infection, extrusion of the silicone, and in severe cases, loss of skin, fat necrosis, embolism, and death.

If we look under the microscope, you can see that the granuloma tissue is surrounded by inflamed cells. Silicone granulomas will not go away with time. The treatments, which we will discuss in another video, include injection, excision, and major reconstruction. Think about your health, your family, and your future before going to a hotel and having a madam inject some sort of liquid into your buttocks or any part of your body because you are desperate to have a better body. Although silicone granuloma might not occur immediately and you might be happy for the first couple of years after the injection, studies have shown that complications can occur decades after the procedure. So, if you do not have any problems, great, but most likely, you will develop a problem in the future. The only universally accepted method to change and augment areas of the body is injecting fat.

During your buttock augmentation recovery period, you must take enough rest to heal properly and quickly. Most patients take around two weeks off work.BUTTOCK IMPLANT RECOVERY

Most patients are told to remain continuously in bed for the first 24 hours after the procedure.You can take limited walks from the second day; however, you should not sit down or sleep on your back for at least two weeks. When your plastic surgeon finally allows you to sit down, you must sit on a soft cushion for at two months.

Even though many patients recover enough within a week to resume work, it is recommended that you allow your buttocks at least nine days to heal before returning to work. If you have a desk job and are required to sit down for a longer period, or if your job involves strenuous activities, you may feel uncomfortable. Putting pressure on your buttocks for the first few weeks after the procedure is not a good option if you are looking for proper and fast recovery.

After the surgery, your plastic surgeon will assess your recovery and overall health, and if satisfied, he or she will let you go home after a few hours. You will receive detailed advice on how to care for your augmented buttocks. You may also get prescriptions for certain medications that you should take according to your surgeon’s instructions.

When you finally start to sit on your buttocks with support, make sure not to sit down for a long period of time because this increases the risk for dislocation of the implant. However, from the second month onward, you can sit down like a normal person.

If you want to recover properly, it is important to avoid driving without a cushion for the first two months after the buttock augmentation procedure. If it is a short drive, you can place a thick towel under your knees and place another towel under your rear. This will ensure that you can sit down and drive the car with comfort.

During the recovery period, you may also find it hard to bend over. Make sure to keep your necessary things handy and within reach. You may also need someone to drive you home from the hospital and accompany you at home for the first two days.

Instead of sitting down, it is recommended that you lie on your stomach for the first few days.Even eating, drinking, sleeping, and reading should be done in this position, which can be a very uncomfortable experience. If you see or feel anything abnormal or think that there is something hindering your recovery process, you should contact your surgeon immediately.

As soon as the anesthetic wears off after the buttock augmentation surgery is performed, the patient will be able to communicate shutterstock_269447099with the medical staff. Many patients say they have felt tired and a bit confused after the surgery, but it gets better in less than twenty minutes when they become fully aware. There is no pain or other unpleasant reaction after the surgery; the patient will feel sleepy for a few hours until the anesthetic is completely out of his or her system.

After this period is over, the patient will feel tension in the area where the surgery was performed that will last 24 to 48 hours. It is possible also to feel some pain during this time, but the intensity will be reduced and easily controlled by medication.

It is recommended that the patient have someone to pick him or her up from the clinic, and the medical staff will provide a list of prescribed medications as well as a list of instructions to follow once they get back home. The first days at home can be hard due to the difficulty moving around, all the bandages, and the elastic shorts. During these first days at home, the recommendation is to monitor the patient’s general health and report anything disturbing to the doctor or the clinic.

Complications after this surgery are extremely rare to nonexistent, but there are other effects that are completely normal and can occur in some cases. For example, when changing the bandages, bleeding can occur. After the buttock augmentation procedure, the area will be swollen, and some inflammation might occur. The swelling can disappear in a few weeks, but it is just as normal to take a few months.

The recovery period after the buttock augmentation procedure might take longer and be a little more painful than for other surgeries, due to the daily demand on the area while sitting down (even using a cushion), walking, and even standing. The patient will need to sleep on his or her stomach or side to avoid unnecessary pain and pressure on the buttocks. It is also normal for the swelling and bruises to make their way up toward the abdominal area or down the hips; this is all a part of the recovery process, and they will disappear in a few weeks in most cases.

Pain medication will be prescribed, and direct pressure on the buttocks should be avoided for about twelve weeks, so prepare to use a pillow every time you want to sit down. If you take care to respect the doctor

Most women desire to have beautiful, rounded, and large bottoms. Some lucky women get naturally attractive buttocks, shutterstock_279345794but for those who do not, buttock implants can be a great option. While this procedure is an effective way to add shape and volume to your butt, post-operative care is the key to the sustainability of your butt implants.

Your surgeon may tell you to avoid sitting down for a few weeks after buttock implant surgery. When you are finally allowed to sit down, it may feel different from what you used to experience with your natural buttocks. No doubt, adjusting to your new buttocks will take time.

Whether you can feel the butt implants depends on the location and size of the incisions and how well your plastic surgeon has performed the surgery. It is common for butt implant patients to experience intermittent or continuous pain and heaviness in the buttocks for the first two months.

When sitting down after butt implants, you will feel like you are sitting on a filling. You will probably experience this feeling in the upper part of your buttocks, even though this is not the pressure-enduring area when you sit down. Buttock implants are normally located higher in the gluteal pocket. When you sit down, you are not actually sitting on the butt implant itself.

However, as time passes, your body and skin make gradual adjustments, and you will likely forget about any strange feelings three months after the procedure; your buttocks will be largely back to normal and feel soft and natural.

Your plastic surgeon

It is important to understand, however, that the butt implant recovery period comes with inconveniences. You will be told to take precautionary measures like not sitting down for a few weeks or days. You should abide by your surgeon

In order to avoid discomfort when sitting down for the first time after butt implants, make sure to place a soft cushion beneath your bottom. You may feel as if you are sitting on padding or a wallet. If you are uncomfortable with the feeling, lie down on your stomach. There are also some medications your surgeon may prescribe to minimize the pain and discomfort when sitting down. Make certain to take your medications on time and according to your surgeon

Feeling the butt implants when sitting down also differs from one patient to another. Some people may feel heaviness in their buttocks, while to others it may seem like their buttocks have become harder. Do not fear these feelings, because they will gradually subside. However, if you still feel uncomfortable while sitting down four months after buttock implants, you should consult with your surgeon.

We earlier wrote about the ideal candidate for a Brazilian butt lift, one of the most popular body contouring procedures last year, not just here at our Houston buttock enhancement practice but in the entire country as well.

Apart from wanting to know how it’s like to prepare for the procedure and whether or not they’re an ideal candidate, another common concern from our patients wanting to have a Brazilian butt lift is how recovery would be like.

Here’s a quick glimpse on how recovery after your Brazilian butt lift would be like:

  • Sitting may not be allowed during the first two to three weeks following surgery with the exception of toilet breaks. By and large, you will be instructed to lie on your side or stomach while sleeping, reading, or doing similar activities. The same approach should be utilized while you’re being transported from the surgical facility to your home. The rationale behind this instruction is that you need to give the fat grafts enough time to incorporate with the underlying blood vessels and consequently build their own network of blood supply.
  • If you’re thinking of stretching or squatting post-surgery, they may not be a good idea at all as they can put significant wear and tear on the skin and tissues.
  • Compression garments may be put in place immediately after the procedure. You will be given instructions on how to take care of them as well as the best time to have them removed which is typically within a week or two.
  • Swelling, alongside pain (particularly once the anesthesia wears off), will be present following surgery. In some cases, swelling can even last for weeks or months. Desired outcomes will most likely be noticed once the swelling has subsided. Pain medications will also be prescribed.
  • Drains will be put in place during the first few days to help eliminate buildup of fluids and reduce swelling at the same time. Oral antibiotics may be prescribed to help reduce your risk of postoperative infection.

I always recommend my patient to be active after Brazilian butt lift. You should stay in the area where you have your procedure done for at least 7 days to make sure that everything is going well, your pain is well controlled and that you are in good condition to be on a flight for such a long period of time. Clots do not typically occur on healthy patient after surgery unless the patient was not active after the procedure. Immobility after surgery increases the risk of clots after surgery due to pooling of the blood in the larger vessels of your leg setting the stage for a cascade of event that result in clots. Most patients after a Brazilian butt lift do not complain of significant pain. If your pain threshold is low then this needs to be address before your flight so you can move around without significant problems.

Anticoagulation in a healthy patient is currently controversial. There is consensus by most surgeons that patient with high risk for clots should have anticoagulation prophylaxis. On the other hand, cosmetic patients usually do not have significant risk factors to justify anticoagulation. Nevertheless, I sometime use these medications (Lovenox) on a patient to patient basis. For example, if I notice that the patient is not very active, pain threshold is low preventing the patient from moving, long flight after surgery etc, I will suggest anticoagulation. Remember it can be a catch 22 because anticoagulation can prevent clots but also increase the chances of bleeding.  In addition, anticoagulation needs to be done for two weeks after surgery since it is well know that you can develop a clot even after 10-14 days after surgery.

After a Brazilian butt lift, there is a period of no sitting of about two weeks. After two weeks, you should be able to drive with a cigar shape cushion under your thighs to bump your buttocks and avoid direct pressure on the newly transferred fat. This will help optimize your end results. In my practice in Houston, I routinely recommend this post surgical protocol.
